In PT3 click Database --> Database Management, then click on the database you want to remove and click "Delete". NOTE: You cannot undo this action once you have done it. So make sure you really want to delete this database before you do so.

Yes, this will delete the database from disk.

Are you using Access or Postgres databases in PT2?

Please be sure that you want to delete them as this is not recoverable.

First remove them using Maintain Database Names, then use the Utilities -> PostgreSQL Database Functions... to delete them.

The other option is to use pgadmin. Just select the DB, right-click, and "delete/drop". PT2 (or 3) will still have an entry that you should get rid of.
