I had the same problem some time ago. I ultimately had someone from PT support team remotely examining my system to figure out what was going on.
I turns out that you have to wait a certain amount of time between closing the Poker Client (PokerStars, FullTilt, etc) and stopping the hand import process. It was explained to me that if you don't allow a period of 5 to 10 minutes between these actions, the hand histories get locked and aren't cleared from from the HH folder. That's why they are imported again in the next session. It's unclear to me wether this happens only with some systems or if everyone should observe this behaviour.
So for the shorter answer (that at least worked for me):
When you're done playing, close the poker client (e.g. PokerStars); wait 5 to 10 minutes; hit "Stop importing hands" or just quit PT4. The next time you import hands you shouldn't get duplicates anymore.
